In this role, you'll follow a structured AN-ACC assessment process that calls on the skills you’ve developed in direct care to conduct clinical assessments for clients. Duties include:

- Conducting desktop reviews of existing care documentation
- Conducting interviews with residents and care staff
- Collaborate with other practitioners in your team to accurately complete assessment tools
- Assessment of needs to make appropriate care recommendations, for funding purposes

Travel will be required both across the region and interstate at times, with all meal and travel allowances provided.

**What you bring**:

- 5 years’ experience in aged care or a related area of health care
- Relevant tertiary qualifications in Nursing, Physiotherapy or Occupational Therapy
- Current unrestricted General Registration AHPRA
- Demonstrated Australian aged care experience including working with people with dementia
- Demonstrated experience conducting clinical assessments using a range of assessment tools
- Good organisational skills and the ability to work independently and as part of a team
- Plenty of empathy and a real desire to help older Australian live better lives
