**Reporting to: Senior Specialist Support Coordinator**
**Remuneration depending on experience**
**Time Commitment**:Part-time 3 days a week with the possibility of Full-time extension
**Position Purpose**:
The Support Coordinator is responsible for supporting a case load of NDIS participants to implement all the supports within their NDIS plan including: informal, mainstream and community, as well as funded supports. This may include but is not limited to our support coordination participants.
This position is responsible to provide each of their participants assistance to strengthen their ability to coordinate their own supports, provide expertise NDIS information, support participants to build capacity to achieve greater independence and provide the NDIA reports on outcomes and success indicators within the agreed reporting frequency. Home visits to participants are an integral part of this role.
Key deliverables for this role are to provide resources and connections with NDIS services and NDIS information for their allocated NDIS participants as a Support Coordinator.
Our role is very much focused on driving clear participant outcomes. Striving to ensure that quality services and frameworks are put in place for our participants with complex needs that will assist them in achieving goals as set out in their NDIS plans. Your strong stakeholder management skills and ability to help navigate through a process will be critical to your success in this role.
**Key Responsibilities**:
- Providing Support Coordination in line with NDIS Practice Standards
- Providing person-centred services that enrich the lives of people living with disabilities
- Develop service plans to address barriers
- Negotiate appropriate support solutions with multiple stakeholders and seek to achieve well-coordinated plan implementation
- Ensure that all funding management activities are undertaken, and that participants are able to maximise outcomes outlined in individual plans
- Liaise with relevant plan managers and rostering officers to ensure that your clients receive the best possible support in an accurate manner
- Liaise with NDIA, NDIS Quality and Safeguards Commission, and other key stakeholders as required
- Ensure that all participant files are up to date, complete and are of a high standard
- Ensure all mandatory reporting is undertaken and required assessments completed within timelines and of a high quality
**Skills and Competencies**:
- Experience working with support coordination clients
- Experience in the disability sector and great knowledge of the NDIS system
- Self-starter and have the ability to work independently under pressure in a fast-paced environment
- Have the ability to work against billable target KPIs
- Respectful and courteous of all people, cultures and lifestyles
- Relevant tertiary qualification (in Psychology, Social Work, Occupational Therapy or other Allied Health disciplines (essential)
- Demonstrated experience a disability setting, including working with participants with complex needs and barriers, including mental health and complex psychosocial disability
- Demonstrated experience and expertise in planning, assessment, referrals and case management implementation
- A thorough understanding of the NDIS, Practice Standards and relevant sector reforms
- High level written and verbal communication skills
- Excellent time management and organisational skills
- Possession of good knowledge of local service providers, both specialist and mainstream, and the ability to search and source services to meet participant goals
- Demonstrated ability to work autonomously and with flexibility including managing competing priorities, using initiative to problem solve and manage own workload
**Job Type**: Part-time
Schedule:
- 8 hour shift
- No weekends

- Smithfield, SA 5114: Reliably commute or planning to relocate before starting work (required)
**Experience**:
- Support Coordination: 1 year (preferred)
Licence/Certification:
- NDIS and Police Check (preferred)
- Driver Licence and access to car (preferred)

- Australia (required)
