**About the Role**

As a NDIS Service Delivery & Intake Officer, your role is to process intake documentation for NDIS participants, care planning and ongoing service delivery. You are a central point of contact for NDIS participants, their family members, support workers and all other stakeholders. This role can be based in our Castle Hill office or in Forresters Beach / Central Coast.

**Responsibilities**
- Coordinate clients’ referrals, assessments, onboarding, care plannings and ongoing service delivery
- Handle NDIS SIL/SDA enquiries, conduct assessments and onboarding process
- Actively promote core support services to prospective NDIS Participants
- Create and continuously develop relationships among Participants, their family members, Support Workers and all other stakeholders
- Forecast and monitor individual participant’s budget to ensure appropriate support delivery within the budget
- Provide accurate case notes and progress reports that reflect forward planning in line with the participant's NDIS plan
- Lead the Support Worker teams to provide responsive and person-centered services

**Requirements**:

- Extensive disability sector experience including in the SIL/SDA space
- Good knowledge of the NDIS rules and the NDIS Quality and Safeguards Framework
- Positive, patient and a passion for helping others
- Excellent communication and interpersonal skills
- Strong organizational, time management & problem-solving skills
- Ability to handle sensitive and confidential information with professionalism and discretion
- Demonstrated ability to work Independently and within a team.
- Relevant qualifications or certifications in disability supports or related fields.
- Understanding of the NDIS including the legislation, relevant policies & procedures.

Pay: From $65,000.00 per year

Schedule:

- Day shift
- Monday to Friday

Supplemental pay types:

- Performance bonus

- Drivers Licence and Roadworthy Vehicle (preferred)
