**2 x Case Manager Alcohol & Other Drugs - 68.4 hours per fortnight**
- **1 x Case Manager AOD Outpost - 76 hours per fortnight**:
- **Multi-site role (Inner Melbourne, Dandenong and Southbank)**:
- **79,149.47 prorata + super + NFP salary packaging**
**What you’ll be doing**
Reporting to the AOD Coordinator, you will provide support to clients of Launch housing who are experiencing addiction and/or dependence. This could range from a one-off harm reduction session, right through to long-term case management. You will also be responsible for contributing to a world-class harm reduction program.
**You will**:

Support a self-determined caseload of voluntary clients to identify and achieve AoD related goals
- Complete AoD screening/assessment tools, and support clients in accessing residential and community-based support services
- Develop Individual Treatment Plans (ITP's) collaboratively with clients
- Support complex clients through the life of their treatment plan
- Provide education and training sessions to staff of Launch Housing
- Provide secondary consults to Launch Housing staff and other external agencies

Note **that for **_Case Manager AOD Outpost role, you will be required to be on work rotation across the 4 crisis sites - Southbank, South Melbourne, St Kilda and Dandenong_* as such a full Victorian driver licence is a mandatory requirement.
- For _*_Case Manager AOD position, you will be permanently based at Southbank site.

**What we’re looking for**

passion for working with people experiencing homelessness and a determination to deliver innovative and effective services of the highest possible standard to our clients
- tertiary qualification in Social Work, AoD or related field
- previous experience working with people using alcohol and other drugs with a commitment to harm reduction practices
- an understanding of trauma-informed practice and both the individual and structural causes which lead to addiction and dependence
