**Position Overview**:
As a Disability Support Worker, you will play a vital role in delivering high-quality, person-centered care and support to our clients. You will be responsible for assisting clients to navigate the NDIS system, providing personal assistance, supporting community participation, and facilitating accommodation needs.

**Key Responsibilities**:

- Personal Care & Support: Assist with daily activities such as personal hygiene, dressing, feeding, and mobility, promoting dignity and independence at all times.
- Social & Community Participation: Support participants in accessing community-based activities and opportunities that align with their interests and goals, promoting social inclusion and engagement.
- Developing & Implementing Care Plans: Work collaboratively with participants, their families, and other support professionals to develop individual care plans and assist with the achievement of personal goals.
- Health Monitoring & Wellbeing: Administer medication, monitor health conditions, and ensure that participants’ overall well-being is managed effectively. Liaise with healthcare professionals when required.
- Documentation & Reporting: Maintain accurate and detailed records of care provided, progress, and any incidents, in line with NDIS guidelines and legal requirements.
- Team Collaboration: Work alongside other support workers, therapists, and healthcare professionals to ensure coordinated, person-centered care

**Essential Qualifications and Skills**:

- Certificate III or IV in Disability Support (or equivalent)
- Previous experience in disability support, aged care, or a related field (highly desirable)
- Strong interpersonal and communication skills, with the ability to build rapport with participants and their families
- A compassionate, patient, and empathetic attitude towards people with disabilities
- Ability to work independently and collaboratively within a team
- First Aid and CPR certification (preferred or willing to obtain)
- A valid National Police Check and Working with Children Check (or willingness to obtain)
- A current driver’s license and reliable transport (depending on the role and location)

**Job Types**: Full-time, Permanent

Pay: $72,000.00 - $80,000.00 per year
