**About the role**
- The Prevention and Population Health Advisor, may be located in or relate to any of the geographic regions, will lead, coordinate, support, enact and develop work to consolidate, progress and report work done across GPHU PPH teams, and externally in partnership with agencies and officers inside and outside the health sector. While the work a PPH advisor may differ from counterparts located in other parts of our region, an Advisor brings PPH skills which are critical to the success of interventions designed to address evidence of health deficit or inequalities across the catchment.

**Key Responsibilities**
- Coordinate one or more Prevention and Population Health (PPH) streams as required within the GPHU, acknowledging that a strong PPH team is often multidisciplinary and will likely have diverse membership. Streams may include but not limited to physical activity, healthy eating, and reducing harms from tobacco and e-cigerettes.
- Coordinate with other GPHU team members as needed to ensure strong stakeholder engagement is maintained to achieve Stream goals.
- Strengthen PPH Stream activities across the region by coordinating and supporting collaboration, developing (where possible) shared process, impact and outcome (short and long term) evaluation measures, and ensuring any internal and external reporting requirements are met when due.
- Develop, build and maintain cohesive working relationships with key stakeholders and partners to effect the planning, delivery and robust timely evaluation of health promotion, disease prevention and population health initiatives.
- Map and monitor local evidence-driven and/or Department of Health-priority prevention and population health streams.
- Undertake other tasks as required as part of a broader team.

**Skills and experience**
- A tertiary qualification in Population and Public Health or relevant stream field, or equivalent relevant qualification and experience
- Sound knowledge of public and population health risk factors and disease epidemiology and their social determinants drivers in the Grampians region
- Demonstrated success in leadership in health promotion, disease prevention or population health (or a related endeavour) such as project governance, design, planning, implementation, monitoring and reporting
- Recognised decision-making skills to solve unforeseen problems which threaten program outcomes
- Excellent communication (written and verbal) and documentation skills

**Work Environment**
- Experience in initiating and managing quality relationships with community partners, sector stakeholders, community groups and leaders
- Ability to oversee or contribute to the formulation, implementation and on-going review of staff training programs
