**Comment**:
This role will require working in a diverse range of settings including but not limited to, a person’s home or place of residence, other services, community groups and other community settings.
**1. AGENCY STATEMENT**:

**2. PURPOSE OF THE POSITION**:
The Disability Support Worker will provide individual support to people with disability, using a flexible, responsive and individualised approach to assist participants to meet their individual and articulated goals.
**3. SCOPE**: The Disability Support Worker will:
- Provide high quality support that addresses client needs as articulated in their Plan and enhances their independence, abilities, community participation and quality of life.
- Respect the personal preferences of clients in meeting their needs and encourage individual growth and development.
- Work in conjunction with other Support Workers and management to perform duties as directed.
**4. DUTIES AND RESPONSIBILITIES**
**4.1. Specific**
- Support individuals to be valued members of their community, through assisting each person to adopt valued roles, form meaningful relationships, access community activities, venues and services.
- Participate in developing and monitoring individual plans and be accountable for their outcomes.
- Assist in the delivery of programs that facilitate community inclusion and participation, promoting individual needs, abilities and preferences.
- Promote community inclusion through actively seeking community-based opportunities for individuals based on their interests, needs and cultural requirements.
- Assist individuals to identify appropriate and achievable goals as well as strategies to achieve these, ensuring dignity, privacy and confidentiality always.
- Where required, assist individuals to undertake a range of activities of daily living including personal and self-care, eating and maintenance of personal hygiene, using the principles of active support.
- Support individuals to use their preferred method of communication and assist them to be understood, always listened to and responded to.
- Work effectively with people who have high physical needs and/or behaviours of concern implementing manual handling assessments and/or behaviour support plans as required and in keeping with Umbrella Disability Services procedures and policies.
- Ensure that all programs and service delivery are linguistically and culturally appropriate to the needs of the people we support.
- Maintain documentation, records and data collection as required and in accordance with the relevant legislation and Umbrella Disability Services policies.
- Interact and communicate with other team members in the interests of service users and of achieving a consistent approach to service provision.
**4.2 General**
- Comply with Federal, NSW and Local Government legislation, regulations, permits and/or by laws.
- Adhere to Umbrella Disability Services Policies, Procedures and other general conditions of employment.
- Comply with Service Agreements, operational guidelines, approved work plans, reporting requirements or task directives.
- Adhere to budgets, administrative and data collection ensuring all procedural requirements are met in a timely manner.
- Comply with Umbrella Disability Services Work Health & Safety (WHS) requirements and policies whilst also remaining vigilant in relation to any participant or worker related behavioural risk and contribute to maintaining a safe work environment.
- Maintain confidential client, personnel and organisational information in line with legislative requirements and Umbrella Disability Services policies and procedures.
- Work collaboratively with Umbrella Disability Services' staff and management to address service improvement requirements resulting from client complaints, stakeholder feedback and/or internal or external evaluation processes.
- Actively participate in supervision, performance reviews, professional development activities and training as required.
- Maintain time, attendance, leave and duty records in accordance with Umbrella Disability Services procedures and lodge these within specified timeframes for each pay period.
- Sign and adhere to the NDIS Code of Conduct
