About the role
- The purpose of this position is to deliver Social Work clinical services to Health Service consumers and their families/carers, with a focus on optimising psychosocial outcomes to maintain, promote and improve quality of life.
- This position may include providing quality Social Work services in a rehabilitation, aged care, interim care, community acute/post-acute or chronic disease management context. 
- Social Work positions in Community and Oral Health are based in community health teams, residential facilities and subacute care units.
- Standard working hours are from 0800 to 1630 hours Monday to Friday, excluding public holidays, and with some variances in start/finish times dependent on the clinical alignment. Employment within Post-acute Care and Hospital in the Home may require rostered weekend work including public holidays with extended hours. Upon request - this role may be required to work weekends in other services.

About you

**Mandatory requirements**:

- Appointment to this position requires proof of qualification and registration or membership with the appropriate registration authority or association. Certified copies of the required information must be provided to the appropriate supervisor/ manager, prior to the commencement of clinical duties.
- This position requires the incumbent to operate a 'C' Class motor vehicle, and an appropriate Queensland licence endorsement to operate this type of vehicle is a mandatory requirement. Proof of this endorsement must be provided before commencement of duty.
