The roll:

- Manage company employee and customer files.
- Assist case corodinators in customer management, including arranging nursing support, nursing services and various other services required by other customers.
- Assist case managers in working with multiple health service teams to provide customer support and meet customer needs.
- Maintain good communication and collaboration with customers, families and service personnel.
- Assist care coordinator to undertaking a comprehensive assessment of each client in collaboration with the client and their family or carer
- Determining health and wellbeing maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life
- Coordinating the safe delivery of quality holistic care to the client and monitoring client progress
- Engaging with allied health professionals and community supports to enhance client care delivery
- Developing associated documents and reporting, as required

What you will need:

- Relevant experience or background in healthcare, social work and community services, and disability services is required.
- Basic knowledge of home-based care and NDIS services is required.
- Must be proficient in Cantonese and Mandarin and fluent in English.
- Valid Australian driver's license is required.
- Excellent written and verbal communication skills are required.
- Excellent time management skills are required to complete work tasks in a timely and efficient manner.
- Strong problem-solving skills are required.
- Proficiency in Microsoft Office software and strong ability to learn computer software are required.

**Job Types**: Full-time, Part-time, Permanent

Pay: $60,000.00 - $80,000.00 per year

Expected hours: No more than 38 per week
