**About the role (FEMALE ONLY)**

**Heartfelt Personalised Support** is seeking a compassionate and dedicated Disability Support Worker to join our team in Victoria. As a Disability Support Worker, you will play a vital role in enhancing the lives of individuals living with disabilities, providing personalised care and support to help them achieve their goals and lead fulfilling lives. This full-time position offers the opportunity to make a meaningful difference in your community.

**What you'll be doing**
- Assisting clients with daily living activities such as personal care, meal preparation, and household tasks
- Facilitating community access and participation, accompanying clients to appointments, social activities, and recreational outings
- Advocating for client needs and collaborating with other healthcare professionals to ensure holistic support
- Implementing personalised support plans and documenting client progress
- Maintaining a safe and inclusive environment, adhering to all relevant policies and procedures
- Providing emotional support and empowering clients to reach their full potential
- **Can manage and handle psychosocial clients**

**What we're looking for**
- Demonstrated experience in providing person-centred support for individuals with a range of disabilities
- Strong communication and interpersonal skills, with the ability to build trusting relationships with clients
- Empathy, patience, and a genuine commitment to making a positive difference in the lives of others
- Relevant qualifications, such as a Certificate III in Individual Support or equivalent
- **A valid driver's license and access to reliable transportation (important)**:

- First aid certification and a willingness to undertake ongoing training
