About the role
We are seeking a highly motivated Care Consultant to join our team in Bendigo and the Surrounding Area.
As a Care Consultant you will spend 70-80% of your day visiting clients, assessing the ageing related needs of the customers and facilitate necessary supports and services within the limits of their individual home care budget and the scope of the funded program to ensure that customers can live safely and independently in their own homes.
**This role is responsible for the following**:
Intake, initial and ongoing assessments
Initial and ongoing service planning
Case management and goal directed care planning
Regular/ongoing home visits and monitoring
About you
**To be successful in this role, you will need the following**:
Qualification in Case Management, or
Undergrad qualification in Nursing / Allied Health / Social Work is essential
Div 1 Nursing Qualification - desirable but not essential
Prior case management experience will be highly regarded
Experience completing initial and annual customer assessments and care planning
Knowledge of the Home Care Package industry
Must be willing to undertake a police check
