**ABOUT YOU**
We're looking for a **Permanent Part Time Case Manager** to join our team, working **Tuesday to Thursday, 8.30am to 5.00pm**.
In this vital role, you'll deliver person-centred care by supporting clients through every stage of their journey, from consultation and intake to care planning and ongoing wellbeing monitoring. You’ll be a trusted advocate for your clients, building strong relationships and ensuring their needs are met with compassion and professionalism.
You’ll work closely with our service coordination team to make sure your clients receive the comprehensive, tailored support they deserve.

You’re proactive, empathetic, and driven to make a genuine impact in people’s lives.

You’ll bring:

- Experience in case management within community services
- Excellent communication and interpersonal skills
- Proven ability to work independently, exercise sound judgment, and manage competing priorities in a fast-paced environment
- A warm, client-first approach to service delivery
- Strong computer literacy and information management skills
- A solid understanding of risk management, complaint handling, and incident response
- A qualification in Nursing or Allied Health (highly regarded)
- A current driver's licence, reliable vehicle, and comprehensive insurance
