HomeCaring, a NDIS registered provider, is a team of home care professionals that is dedicated to offering comprehensive and compassionate care in disability, aged care, and dementia care, as well as helpful support services, to all clients who come to rely on us.

This position is a key member of the Services Team providing assessment, care planning, monitoring and referrals for all clients. The Case Manager is responsible for performing in home face to face assessments and partner with clients, their carers/families to assess current situations and create case plans that enable maximum independence and positive impact for the person. MUST drive.

**ESSENITAL REQUIREMENTS**:

- NDIS Worker Clearance
- Current National Police Check
- Working With Children's Check
- Australian Driving License
- First Aid & CPR
- Relevant case management experience supporting clients with high complex needs
- Successful completion or working towards a diploma/degree/post grad in Social Work, Social Welfare, Psychology or related discipline

**Main responsibilities and activities will include**:
**1. Person centred assessment and care planning**
- In consultation with clients, carers and families facilitate care assessment, care planning and review
- Develop and monitor individual plans and budgets with regular reviews/case conferencing
- Manage any identified risks within principles of Positive Approach to Challenging Behaviours and Duty of Care, Dignity of Risk
- Development and maintenance of all documentation in client management systems
- Work within legislated NDIS and CDC frameworks
- Be aware of and comply with legislation and organisational requirements relating to Workplace Health and Safety
- Contribute to quality processes such as management of client feedback
- Actively participate in professional development activities (internally and externally) to maintain contemporary knowledge and skills as well as acquire new knowledge and skills

**2. Client and stakeholder engagement**
- Make relevant referrals, broker services and deal with suppliers as per care plan
- Develop and maintain relationships with community organisations to ensure a holistic case management approach
- Develop and maintain relationships with brokerage agencies to ensure quality services are delivered by external providers.
- Ongoing communication with clients, carers/families to ensure they are aware of rights and responsibilities to make informed choices.
- Participate in on call roster

**3. Business Development**
- Ongoing relationship management with relevant referral sources
- Develop links in the community to source new referrals
- Retention of existing consumers
- Participate in objectives and performance review to reach KPI’s set in relation to new and existing consumers.
