YourBridge Cares is expanding our support team for a participant living in a Supported Independent Living (SIL) home. We are seeking **male support workers** who are confident, patient, reliable, and passionate about helping participants achieve meaningful daily outcomes.

We are a Bridge of Hope to people experiencing mental illness or disability. Our mission is to provide uncompromised support, dignity, and encouragement so participants can overcome obstacles and move toward recovery and independence.

**Purpose of the Position**

Our Support Workers follow each participant’s Individual Support Plan and deliver high-quality, person-centred support. You will be working in the participant’s SIL home and in the community - shopping, appointments, recreation activities, library, healthcare facilities and other community settings.

Your role is to strengthen the participant’s capacity, independence, emotional wellbeing, and community engagement.

**Key Responsibilities**
- Provide community access, including attending outings, recreation, exercise, and appointments
- Provide companionship, especially for participants at risk of social isolation
- Support with daily living tasks — light domestic routines, planning, organising, and communication
- Introduce and participate in meaningful activities such as gardening, art, cooking, gaming, music, reading
- Collaborate with families, carers and allied health teams
- Support the implementation of therapeutic strategies and behaviour support plans
- Assist in developing individual recovery plans
- Provide personal care support where required
- Complete daily documentation: support logs, case notes, incident notes
- Follow all organisational policies, NDIS Standards, Work Health & Safety procedures
- Maintain respectful, safe professional boundaries at all times

**Work Standards**
- All support delivered according to the Individual Support Plan
- All support delivered with respect, empathy, safety, and professionalism
- Documentation completed daily and accurately
- Code of Conduct (YourBridge + NDIS) adhered to
- Positive engagement with participant goals
- Policies and OHS instructions followed without exception

**Essential Experience**
- Experience as a mental health or disability support worker
- Ability to work independently and in a team with confidence
- Strong interpersonal communication skills
- Ability to adapt support approach to suit individual needs
- Understanding of recovery-oriented practice
- Experience developing or following support plans
- Ability to follow Positive Behaviour Support Plans
- Understanding of restrictive practices and incident reporting

**Qualifications**

**Required (minimum)**
- Certificate IV in Mental Health, Disability, Community Services or related
- First Aid & CPR
- Valid Driver Licence + reliable vehicle (registered, insured & roadworthy)
- NDIS Worker Screening Check
- Working With Children Check
- National Police Check
- Up-to-date COVID-19 vaccination
- Right to Work in Australia

**Preferred**
- Degree in psychology, social work, community services
- Experience with severe and persistent mental illness
- Experience with SIL or high-needs behavioural support
- Experience working in a multi-disciplinary care team
- Experience supporting participants with AOD, trauma backgrounds, or forensic disability

**Supervision**:
This position will be reporting to the service manager for ongoing support and supervision unless otherwise stated.
