**Purpose of Position**

To assist new and existing Simply Helping Gippsland South and West service users to:

- achieve their goals and receive safe, quality supports aligned to individual preferences through the thorough and efficient completion of service agreements, care plans and associated documents.
- arrange supports and assist service users to effectively manage funding.

**Key Responsibilities**
- Work collaboratively and constructively with Simply Helping teams/departments, external service providers, allied health professionals and other stakeholders to achieve the purpose of the position.
- Receive and promptly process referral forms for new enquiries/referrals.
- Complete the intake process for service users in accordance with Simply Helping processes.
- Liaise with the Clinical Educator and Senior Programs Admin: Complex on any complex care/medication plans or specific training requirements for support workers assisting complex service users.
- Assist service users to understand their allocated program funding and how to optimise the use of funding with appropriate services.
- Assist service users to engage with services in alignment with their goals and monitor and record their progress.
- Assist service users and Simply Helping service coordinators to set up and implement supports.

- Excellent attention to detail.
- Strong workload management ability.
- Clear communication and ability to develop report with colleagues, service users and other stakeholders.
- Good problem-solving and decision-making skills.
- Strong ethical standards and personal integrity.

**Qualifications, Experience and Knowledge**
- Proficient in Microsoft Office.
- Understanding of NDIS/TAC/HCP rules and standards.
- Valid Police Check, WWCC, NDIS Screening Check & 3 Covid Doses required.
