**Role Overview**:
This is an exciting opportunity to join UnitingSA’s team as the Care Connect Officer. This role is responsible for providing accurate and timely information and support to clients regarding UnitingSA’s Community Aged Care service offerings.
Under the management of the Business Development Manager, the Care Connect Officer will respond to enquiries and provide information regarding financial and contractual requirements for Community Aged Care clients including Home Care Packages (HCP), Commonwealth Home Support Program (CHSP) and Fee for Service, to assist clients to access services with UnitingSA.
**Key Responsibilities**:
- Ensure a person-centred approach to understand goals and aspirations for individual clients.
- Ability to interpret Aged Care Assessment Team (ACAT) assessments and how they inform conversations with potential clients.
- Strong understanding of HCP and CHSP guidelines with ability to easily explain how these programs can support potential clients in easy to understand language.
- Provide responsive and professional customer service (both internal and external) relating to enquiries for Community Aged Care services.
- Provides information regarding financial and contractual requirements for potential clients and family members to assist with making informed decisions.
- Ensure that the required administration processes are undertaken.
- Assist in resolving client needs, expectations and goals.
- Communicate comprehensive knowledge of available supports and services.
**Skills & Experience**:
- Knowledge of Community Aged Care services and supports.
- Ability to provide advice to potential clients seeking services and supports from UnitingSA.
- Ability to positively promote the service and the issues pertinent to the client group to other organisations, government bodies and community groups.
- Demonstrated initiative and a proactive approach using good time management skills, being organised and efficient through the use of quality processes.
- Experience in the coordination of service provision in the Community Aged Care.
- Demonstrated experience and commitment to delivering great client experiences, focused on meeting the meets of the client.
