- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.**About the role**
- Opportunity to join the interdisciplinary Connected Care team, providing care for clients with chronic and complex conditions in the community. The team provide comprehensive assessments, care coordination, linkages to services and some specialist allied health and medical care. The model of care is responsive and agile to client and organisational needs. Some group work and education sessions with disease management group such as Chronic Heart Failure team. The team includes care-coordinators, nurses, medical staff and allied health professionals including psychology, diabetes educator, physiotherapist, and social workers.-
