**Role Description**

We’re expanding our Support Coordination division and are looking for an experienced and motivated Support Coordinator to join our friendly team.

As a Support Coordinator, your role is all about helping NDIS participants achieve the goals set out in their NDIS plans. You’ll work closely with participants to empower them to direct their own lives, not just their services. This involves assisting them in building a strong network of formal and informal supports and guiding them through complex service delivery systems.

This role is hybrid, offering a mix of working from home (WFH) and 2 days in our office located on North Rd, Ormond. You will also occasionally visit participants in their homes. The position is 20-25 hours per week, with flexible working arrangements available. The role also offers the opportunity to transition from a permanent part-time position to full-time in January. **What You’ll Do**:

- Work with participants to help them achieve their NDIS goals.
- Provide ongoing Support Coordination services in line with participants’ NDIS plans.
- Use a person - centred approach to build participant capacity, choice, and control.
- Develop and implement support plans, monitoring their progress and outcomes.
- Tackle complex challenges and provide creative solutions.
- Follow organisational policies, procedures, and values.
- Stay up-to-date with all relevant legislation and codes of conduct.
- Build knowledge of local services and organisations that can support participants.

**What You’ll Bring**:

- At least 1 year of experience in NDIS support coordination.
- Excellent verbal and written communication skills.
- Qualifications in community services, disability, or related health disciplines (minimum Cert 4).
- Strong knowledge of NDIS rules and operational guidelines.
- NDIS worker screening clearance & blue card.
- First Aid and CPR certifications.
- Great interpersonal skills to build relationships with a variety of stakeholders.
- Permanent Australian working rights.
- Tech-savviness and proficiency with Microsoft Office.
- Ability to work independently and show initiative.

Pay: $40.00 - $50.00 per hour
