**What you will be responsible for**:
The Care Coordinator is pivotal to ensuring the successful delivery of services to our clients. Reporting directly to a Team Leader, your core responsibility will be the organisation, scheduling, and management of support workers to maintain consistent and high-quality support. You will contribute to the team’s success by:

- Coordinating and actioning client service requests and bookings in line with Leora’s policies and guidelines
- Managing feedback, complaints, and incidents from clients and support workers
- Monitoring client funding and renewal processes
- Allocating, coordinating, and deploying rosters in alignment with the SCHADS Award and client budgets
- Building strong relationships through regular communication with clients, support worker, families, and other stakeholders

**Skills & Qualifications**:
To excel in this role, you will bring:

- Previous experience in a related role, preferably within the disability or aged care sectors
- Qualifications in Health, Disability, Aged, or Community Care
- Exceptional communication and relationship-building skills
- A strong ability to problem-solve and manage multiple competing priorities in a fast-paced environment
- Knowledge of the National Disability Insurance Scheme (NDIS) is highly desirable
