Nundah, Australia Lutheran Community Care Full time

Based from our **Our Zion Aged Care Facility located in Nundah, we have a casual opportunity to joing our team for a Support Worker to provide Home Care for our aging clients. Availability required from 6am to 6pm.**

**_ Let’s talk about the role:_**

As a casual Support Worker (Home Care), you will be responsible for:

- Delivering services for clients at home and in the community
- Contributing to assessments of the client’s goals, interests and needs
- Assisting the service coordinator to maintain appropriate communication with carers and family members nominated by the client
- Adhering to risk mitigation procedures and bring new risks to the attention of the service coordinator.
- Advising the service coordinator of significant changes in the client’s capacity and/or if the client requests a different service mix
- Providing domestic cleaning for clients
- Providing Personal Care for clients
- Driving clients to appointments.

**_ Now let’s talk about you:_**

You will be able to demonstrate:

- Current Certificate III in Individual Support
- A high level of effective interpersonal and communication skills
- Your ability to maintain confidentiality and impartiality
- Respect and responsiveness to client needs
- A willingness to work with non-traditional approaches to service delivery for older people
- Current unrestricted Open Drivers Licence for Queensland
