**ACAS Assessor**

- The Comprehensive Assessor will join an innovative team of assessors that comprise the North West ACAS and Hume Regional Assessment Service. As a comprehensive assessor you will be responsible for assessing eligibility for Commonwealth funded programs, including CHSP, Home Care Packages and Residential Care.
- The Aged Care Assessment Service is inclusive of all older persons, recognising and accepting of diverse groups and those with special needs in our community.
**Your new role**:
- We are looking for an experienced clinician in assessing the needs of older people residing at home or currently in a hospital setting.
- Specifically the role would include:
- Participating in the Intake and Triage team, managing referrals coming via My Aged Care and external sources.
- Undertaking comprehensive assessments
- Participating in any supervision of Grade 1 staff or equivalent
- Engaging with a range of internal and external stakeholders, in particular CHSP providers, advocacy services and HCP and residential care providers.
**About You**:
- You have strong clinical aged care experience in health and/or community settings
- You have strong content knowledge of the issues that impact on an older person managing independently in the community
- You have demonstrated knowledge and experience in working with persons with special needs, including those experiencing homelessness, language and cultural barriers, and those living with dementia
