Our Outreach Service at Ramsay Clinic Albert Road is looking for a dedicated clinician to join our dynamic team.

In this rewarding role, you will
- Visit patients in their homes to deliver **individualised, person-centered support**:

- Focus on **activity scheduling, mental state monitoring and wellbeing promotion**:

- Work one-on-one to build **independence, life skills and community connection**:

- Partner with patients on **goal planning** and pathways to recovery

**About You**

- Registered with APHRA (Nurse, Psychologist, OT) or a Social Worker with professional body membership
- Experienced - **minimum 3 years practice**in a clinical role
