About the Role

Join our recovery-focused Outreach Service and bring care directly into the homes of those who need it most.

Our Outreach Service at Ramsay Clinic Albert Road is looking for a dedicated clinician to join our dynamic team.

In this rewarding role, you will

• Visit patients in their homes to deliver individualised, person-centered support
• Focus on activity scheduling, mental state monitoring and wellbeing promotion
• Work one-on-one to build independence, life skills and community connection
• Partner with patients on goal planning and pathways to recovery

About You

To be successful in this role, you will bring a strong clinical skillset, a genuine passion for patient care, and a commitment to Ramsay's values of integrity, ownership, teamwork, and innovation. You thrive in a dynamic environment, communicate effectively, and deliver care with compassion and professionalism.

You will also bring:

• Compassionate, collaborative and committed
• A strong communicator who can connect and engage
• Registered with APHRA (Nurse, Psychologist, OT) or a Social Worker with professional body membership
• Experienced – minimum 3 years practice in a clinical role
