What You Will Be Doing
ACCESS is the first point of contact for information and referrals to a wide range of community health, rehabilitation and specialist services provided by Peninsula Health. (PH)
This position is responsible for ensuring timely referral management and responsive contact with clients connecting them with the most appropriate service, as per client needs and PH service guide criteria.
- Assist with the triaging of referrals upon receipt, as well as own caseload of referrals, to ensure timely management of priority referrals
- Contact consumers by telephone and verify/collect a consumer's personal and demographic information
- Conduct Initial Needs Identification with the specific purpose of clarifying a consumer's needs, suitability for service, eligibility, priority and risk.
- Determine any service fees (where applicable, and in line with local policies) by ascertaining a consumer's income bracket
- Ensure accurate booking of a consumer's appointments (where applicable) and accurate communication of appointment details.
- Document within a consumer's digitised medical record, ensuring documentation fulfils medico-legal requirements
- What You Need
- A relevant tertiary qualification and/or significant experience in the health or community health services sector
- Advanced interviewing and engagement skills
- Proficiency in the Microsoft Office suite, particularly MS Word and other data base software programs
- Demonstrates advanced computer and keyboard skills
- Current National Police Check
- Current Working with Children Check
- Current Victorian Driver's License

- Please refer to salary circular for ALLIED HEALTH PROFESSIONALS (VICTORIAN PUBLIC SECTOR) (SINGLE INTEREST EMPLOYERS) ENTERPRISE AGREEMENT 2021 - 2026 for salary range. Sub-grading will be based on years of experience.
