**About the role**
We’re looking for a Care Coordinator to join our growing Ageing Services Team. In this role, you'll manage a caseload of Commonwealth Home Support Program (CHSP) clients, helping older Australians live with dignity, independence, and connection.

You'll coordinate holistic support services, conduct assessments and reviews, and build trusted relationships with clients, families, and providers. Delivering care that is meaningful, respectful, and tailored to individual goals.

This is a full-time, hybrid position on a contract ending 25 June 2025, with the possibility of extension. The role offers flexibility to work from home, with regular client visits and occasional travel as required.

**Responsibilities include**:

- Support clients with care services that meet aged care standards and program guidelines.
- Organise client intakes, do assessments, and create care plans that promote wellbeing and independence.
- Keep track of client needs and adjust support when things change, referring to other services if needed.
- Work closely with clients, families, and providers to make sure support is tailored and high quality.
- Keep accurate records and meet all compliance and reporting requirements.

**Skills and Experience**:

- Prior experience in a similar role (aged care, health, or community services)
- Knowledge of the Aged Care Act, CHSP Guidelines, and Aged Care Quality Standards (or willingness to learn)
- Excellent communication, coordination, and relationship-building skills
- Empathy, cultural sensitivity, and a genuine commitment to improving the lives of older people
- Strong documentation, time management, and organisational skills

**Requirements**:

- Open drivers licence (desirable)
- NDIS Worker Screening Clearance (yellow card)
- QLD Blue card (working with children’s check)
- NDIS Orientation Module: “Quality, Safety & You”
- Relevant qualification in Community Services, Aged Care, or similar (desirable)
- Australian working rights (with no restrictions)
