Perth Disability Services is a registered NDIS provider in Perth, Western Australia. We are committed to
delivering quality and safe support services to people living with a disability, their families, and carers.
Our aim is to help people with disabilities live comfortably and confident in the community while turning
their dreams into real opportunities. We support children, young people and families, people with
disabilities, older people, and people with mental illness
We are currently looking for full-time disability support worker/Personal Assistant/ Aged OR
Disabled Care workers with minimum 1 YEAR of exp.
Annual Salary offering - $68000-78000
Working in a team, you will facilitate the delivery of quality disability services and achieve positive
outcomes supporting their individual needs and creating a safe environment.
We will be looking for staff who have good availability to cover several shifts as disability support
workers, if you prefer days or nights.

Key Responsibilities:
Support participants to work towards achieving their individual goals.
Support daily living, promoting cooking, housekeeping, hygiene, shopping, Medication administration,
Support participants in building their confidence.
Liaise and collaborate with clients, their community and community & disability support agencies.
Monitor and review the participant's progress through the service and against the support plan.
Utilise positive behaviour strategies
Support clients to participate in age-appropriate activities and tasks, including employment, in-Work in
accordance with the medication administration policy, work health and safety procedures Monitor the
safety and well-being of people with disability and report any concerns to the Manager.
Ensure completion of appropriate documentation. As required, relevant information on disability
Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
Work with clients, family/ carers, relevant case managers and other professionals to support the Liaise
extensively with other sta to ensure the overall smooth and consistent running of the Report regularly to
the Manager

Skills & Experience
Qualification in community/disability/individual support/aged care/ Mental Health and preferably
Experience working with clients with disability & challenging behaviours.
Well-developed verbal and written communication skills.
Current Driver's Licence and reliable vehicle.

Pay: $68,000.00 - $78,000.00 per year
