We are looking for an enthusiastic individual to fill our Aged Care Support Facilitator position. The Aged Care Support Facilitator is responsible for the delivery of Home Care Package services that focus on person-centred support to enhance quality of life and wellbeing of our clients through promoting individual choice that respect the needs, interests and culture of the people that are supported by CatholicCare. This is a permanent full-time position consisting of 38 hours per week.

The salary range for this position is from $41.52/hr to $44.68/hr depending on experience and qualifications + superannuation + salary packaging.

**Our requirements to provide care**
- Tertiary qualifications in aged care and/or at least 2 years relevant experience in aged care including eg dementia, complex care needs, psychosocial care needs
- High level demonstrated experience in case management; comprehensive assessment with clients; developing individual budget and goals that enable clients to achieve their goals and optimal wellness
- High level demonstrated ability to focus on maintaining independence and wellbeing and to identify available resources and develop links within community
