Craigieburn, Australia Cessible Healthcare Full time

**Working at Cessible Healthcare**

Cessible Healthcare is a care service founded with a deep passion for nursing and a mission to improve the quality of care for individuals and communities. Recognising the need to address staffing shortages and strengthen service delivery, we are committed to making a positive impact across the healthcare, home care, and disability support sectors.

Our team understands the challenges faced by healthcare facilities, families, and individuals living with disability. Through our NDIS services, in-home care, and workforce support solutions, we’re dedicated to providing innovative and compassionate care that improves outcomes and enhances everyday life for the people we support.

**About the Role**:
We are currently seeking a compassionate and experienced Support Worker in North Melbourne to provide consistent, high-quality care to a young participant in early childhood living with complex physical and communication needs.

This role involves delivering full personal care, including manual handling, repositioning, and wheelchair transfers, while working closely with the participant’s family and allied health team. Your support helps create a safe, respectful, and responsive environment where the participant can thrive.

You will be someone who can:

- **Provide hands-on support with all daily living activities**, including personal hygiene, dressing, mobility, and positioning using approved manual handling techniques and equipment.
- **Follow the participant’s health, mealtime, and therapy plans**, and work in partnership with their health and allied health practitioners to ensure safe and consistent care.
- **Understand and follow PEG feeding routines**, watch for signs of distress or difficulty during mealtimes, and act promptly to ensure safe and comfortable feeding.
- **Support medication management**, using appropriate aids and routines to help the participant take their medications safely and on time.
- **Adapt to complex and changing needs**, considering the impact of multiple systems, supports, and barriers, and always working in a way that promotes the participant’s comfort, strengths, and dignity.
- **Bring empathy, patience, and professionalism**, and reflect on your own practice to ensure you're supporting the participant in the most respectful and person-centred way possible.

**Roles and Responsibilities**:

- Provide individualised and flexible support that adapts to each participant’s unique needs and aligns with their NDIS goals.
- Uphold the principles of choice and control, ensuring participants have an active voice in how their supports are delivered.
- Work collaboratively with participants and their support networks to develop and implement strategies that promote independence and well-being.
- Build and nurture trusting relationships with participants, advocating for their rights, dignity, and access to community services.
- Encourage participation in community activities, helping participants overcome obstacles and engage in meaningful, fulfilling experiences.
- Regularly review and adjust care plans, staying attuned to changes in participants' needs, preferences, and circumstances.
- Maintain clear and accurate documentation, reporting on progress towards NDIS goals and contributing to data collection for future care planning.
- Collaborate with external agencies, clinical services, and other providers to ensure seamless and coordinated care for participants.
- Engage in continuous professional development and training to uphold the NDIS Code of Conduct and ensure best practices in care delivery.
- Perform other reasonable duties, contributing to a positive and supportive team culture.

**Skills and Knowledge**:

- Empathetic, patient, and supportive nature with a genuine passion for helping participants achieve their goals.
- Excellent communication and interpersonal skills, with the ability to build rapport and trust with participants and their families.
- Experience working with children or a strong understanding of child-safe practices when supporting younger participants.
- Proactive and adaptable, capable of adjusting supports to meet evolving participant needs.
- Strong advocacy skills to ensure participants’ rights and access to services are upheld.
- Ability to work effectively in a team-oriented environment while maintaining individual responsibility and initiative.
- Basic IT skills for data entry and reporting purposes.
- Physical capability to perform the role, including personal care and mobility assistance when required.

**Qualification and Requirements**:

- Worker’s Screening Check (NDIS Worker Check).
- National Police Check.
- Working With Children Check (if applicable).
- Certificate III in Individual Support (Disability, Aged Care) or a similar qualification is highly regarded.
- Previous experience in disability support or a related field (preferred but not essential).
- Current First Aid and CPR certification (required).
