**About the Role**:
Your Local Care Partner is seeking compassionate, reliable, and experienced support workers to provide high-quality in-home and community-based support for clients living with a disability and/or ageing-related needs. The role involves working across multiple suburbs within Sydney, with flexibility required to accommodate a variety of client needs.

**Key Responsibilities**:

- Provide person-centred support aligned with individual care plans and client goals
- Assist with personal care tasks such as showering, grooming, toileting, and hygiene
- Support with domestic duties such as cleaning, laundry, and meal preparation
- Provide assistance with community access, transport, and social participation
- Monitor and report any changes in the client’s condition or environment
- Maintain accurate and timely documentation through the ShiftCare app
- Follow organisational policies, infection control practices, and safeguarding procedures
- Respect cultural diversity, dignity, and individual choices of clients
- Engage with clients in a respectful, compassionate, and empowering manner

**Essential Criteria**:

- Current First Aid and CPR certification
- Valid NSW Driver’s Licence and reliable, insured vehicle
- NDIS Worker Screening Check and Working With Children Check
- Confidence using mobile apps for documentation and rostering

**Desirable Criteria**:

- Experience supporting individuals with mental health, dementia, or high-intensity needs
- Manual Handling and Medication Assistance certificates
- Knowledge of NDIS and aged care sector regulations
- Minimum Certificate III in Individual Support (Aged Care or Disability) or equivalent
- Multilingual or culturally aware applicants encouraged to apply
