**The Opportunity**

**Responsibilities include;**
- Case Management involving the monitoring of client progress and evaluating care plans in liaison with GP’s and the multidisciplinary team;
- Ensure clear communication between services, preventing duplication of referrals and service delivery;
- Provide a contact point for all client care issues during the episode of care;
- Carry out research and development, providing input to the development of clinical guidelines and participating in appropriate committees;
- Facilitate streamlined and timely access for clients to the community-based services they require.
- The Role is Part time (0.2 EFT)
