This role is involved in the provision of comprehensive assessment of clients, care coordination and self-management coaching support to assist clients to achieve independence and lifestyle goals. This includes support and education of both client and carer to assist in informed decision-making processes and where necessary undertaking an advocacy role.

**Position Status**: Part Time (0.6 FTE)

**Classification**: RN G3B Yr1-2

The Care Coordinator is involved in the provision of expert nursing care to allocated Heart Failure clients, advocating for clients providing health education, promoting health, assisting clients with goal achievement and working with other health care providers to ensure coordinated care for clients.

**KEY SELECTION CRITERIA**:

- Experience in Acute Nursing
- Relevant postgraduate qualifications or prepared to work towards and obtain same
- Demonstrated knowledge and skills in Chronic Disease client self-management, Health Behaviour Change, Client led Goal directed care planning, person centred care and ability to work within an Active Service Model framework
- Demonstrated ability to use electronic communication, software systems and relevant hardware
- Demonstrated well-developed communication and interpersonal skills
- Demonstrated ability to articulate clear decision making processes
- Possess knowledge of relevant standards e.g. National Standards
- Ability to work as an autonomous professional as well as working collaboratively within a multi-disciplinary team environment across the community settings
