Annecto - the people network is an independent, not for profit association committed to increasing opportunities and choices for people with a disability, older people and their carers. **What’s the Job?** The role of Western and Grampians Coordinator - Home Care exists to provide leadership, support and guidance to the Western & Grmapians Case Management Team. The incumbent will ensure that a high quality service is provided to people accessing the Home Care Packages program. **What we need from you**: > Understanding and commitment to implementing Consumer Directed Care Initiatives (CDC) and Person Centred approaches > Knowledge of community resources/professional services which support individuals within the Western & Grampians Region > Demonstrated ability to work effectively with a culturally diverse group > Demonstrated ability to work autonomously with a high degree of initiative > Excellent communication skills, both written and verbal > Demonstrated experience in working with people at all levels in a medium to large organisation with multiple sites > Relevant tertiary qualifications in health/social science > Current Victorian drivers licence > Be vaccinated against Covid 19 including Booster Vaccination #3 > Possess an NDIS Worker Screening Check Clearance or be willing to obtain one You should be eligible to work in Australia. - Annecto is a child safe and equal employment opportunity employer.
