**What You Will Be Doing**

The Grade 2 Dietitian in the At Home Subacute program will provide services to patients requiring rehabilitation, geriatric evaluation and management, neurological, orthopaedic, reconditioning, and palliative care within home and community settings. Responsibilities include delivering flexible care through home visits, clinic visits, and telehealth consultations. The Dietitian will work collaboratively within a multidisciplinary team to ensure optimal outcomes, focusing on client-centred goal setting, timely care delivery, and planned discharge. The role also supports the Hospital in the Home and Home-based Cancer Care programs. Key responsibilities include providing quality and safe Dietetic services, building professional skills, offering clinical supervision to Grade 1 staff, and demonstrating positive workplace behaviours and relationships.

**What You Need**

We anticipate you will have:

- Current Victorian Driver’s licence
- Current Accredited Practising Dietitian
- Ability to competently perform high quality Dietetic assessment, treatment and patient management, including thorough and appropriate discharge planning
- Demonstrated experience and competence with neurological and general rehabiliation
- Well-developed interpersonal and communication skills
- Demonstrated experience in the supervision of Dietetic staff and students (desirable)
- Relevant post-graduate qualifications or working towards (desirable)
