About the job
4D Health and Performance is seeking an enthusiastic and goal-driven ESSA Accredited Exercise Physiologist to be part of our dynamic, professional and passionate team based in Campbelltown and the South Western Sydney Region.
4D Heath and Performance is a dynamic multidisciplinary practice, providing physiotherapy, chiropractic, massage, exercise physiology and rehabilitation services. Due to an increasing demand for our services in the South West Sydney region we are looking for a new team member to assist in the growth and the development of our business.
You will assume responsibility for an existing workers compensation load and future patient referrals servicing the Campbelltown and South West Sydney areas. On a daily basis you will visit patients in various locations throughout the South West Sydney area.

Duties & Responsibilities:
You will service the specific geographic area of Campbelltown and South West Sydney
You will provide assessments, develop an exercise prescription with a strong focus on sustainable outcomes
You will have a thorough understanding of complex sports injuries and musculoskeletal pathology, progressions of rehabilitation, chronic disease management, and the workers compensation system
Case Management of patients
Liaising with a multidisciplinary team to ensure optimal outcomes
Develop networks and marketing for the business
High level written and verbal reporting
Attending case conferences and negotiating return to work outcomes

**Requirements**:
ESSA Accredited Exercise Physiologist and WorkCover (SIRA) Accredited
Previous experience in the Workers Compensation field is essential
Capacity to be able to travel and provide rehabilitation services within the Campbelltown and South West Sydney areas
Ability to work independently and as part of a multidisciplinary team
Excellent exercise prescription skills
Chronic and metabolic disease management
A sound knowledge of rehabilitation of general injuries and post-surgical protocols
Sound knowledge of WC and Dr letter reporting
Excellent verbal and written communication skills
Current NSW driver’s license and reliable vehicle
