**Welcome to Monash Health Residential Aged Care Services**

Monash Health recently undertook a revamp of its leisure and lifestyle program that is delivered across all Monash Health Residential Aged Care and the Transition Care Program.

Due to commence in late 2025, we are currently recruiting for a Full Time Leisure and Lifestyle Team Leader. This is an ongoing position based across the new Boollam Boollam Aged Care Centre, Chestnut Gardens Nursing Home and the Transition Care Program.

This new program supports a seamless transition into the new centre, ensures equity in service delivery, and enhances the resident experience across all Monash Health residential aged care services sites.

**About the Role**:
Reporting to the Facility Manager at Chestnut Gardens and the Boollam Boollam Aged Care Centre this pivotal leadership role ensures the delivery of consistent, high-quality lifestyle programs.

The new Boollam Boollam Aged Care Centre boasts state of the art facilities purposely built for our residents to ensure they receive the very best wellbeing activities through our lifestyle program. Amenities at the new centre include:

- A cinema
- Community garden
- Domestic kitchens on every floor/ward
- In house hairdresser
- Individual living spaces within their house

You will also work in close collaboration with the Transition Care Program and the Manager of Allied Health Residential Services to support continuous improvement.

You will lead a team of Grade 2 Leisure and Lifestyle Assistants, overseeing the design, delivery, and evaluation of engaging, person-centred activities that enhance independence, wellbeing, and community connection.

**Key Responsibilities**:

- Developing and implementing resident-centred lifestyle programs
- Providing governance and supervision for Grade 2 Leisure and Lifestyle Assistants
- Monitoring the quality and effectiveness of programs
- Ensuring alignment with Aged Care Quality standards
- Lead, roster, and support Lifestyle Assistants in delivering 7-day programs
- Supervise and mentor Leisure and Lifestyle Assistants, manage performance, and contribute to workforce planning under the direction of the Facility Managers
- Drive program development that fosters inclusion, cultural connection, and positive risk-taking
- Collaborate with Nursing, Allied Health, and Support Services to design integrated care programs, including a new dementia-focused sundowning initiative
- Ensure programs meet the Aged Care Strengthened Standards, supporting quality of life and meaningful engagement
- Facilitate resident-led initiatives and special events, ensuring emotional, spiritual, and psychological wellbeing
- Review and enhance lifestyle procedures to ensure safe, consistent practices

**About You**:
You will bring proven Leadership experience which will be complemented by your ability to motivate others effectively, as well as your experience in aged or community care, passion for person-centred and inclusive care, strong coordination and communication abilities, and your capacity to work hands-on across multiple sites.

Experience in leading and contributing to quality improvement or research projects, including supporting team members, along with a strong track record in designing and delivering evidence-based professional development and education will see you excel in this role.

You will also possess;
- Valid NDIS Clearance and completion of the NDIS Worker Orientation Module (essential)
- Valid police check clearance including a renewal every 3 years (essential)
- Holds Cert III in Aged Care (or equivalent) and has twelve months experience (full time equivalent) as a Lifestyle Assistant Grade 1 OR
- Holds a Cert IV in Leisure and Health (or equivalent) and has six months experience (full time equivalent) as a Lifestyle Assistant Grade 1
- Current Victorian Driver’s Licence
