As a deeply grounded NDIS and Aged Care provider, we require Support Workers on a casual premise with a view towards permanency. One should be adaptable and flexible to work with individuals with diverse special needs and to help frail, older people, including the prematurely aged, living in the community to maximize their independence.

**About the Role**

**Deivian Care is looking for Support Workers in Burwood, Surrey Hills and Wheelers Hill (VIC) and surrounding areas **with expertise helping vulnerable and marginalized persons through high-quality case management, social work, and outreach. You will play an important role for many participants who may be excluded from mainstream healthcare, social support, and opportunities, negatively impacting their functioning and independence in the community. You'll use evidence-based, integrated, and person-centered approaches to give individualized assistance using clinical frameworks and modalities to create co-developed realistic recovery programs that are both individualized and powerful. Your goal is to help and encourage people to recognize their own self-efficacy and make long-term changes by taking control of their own journey.

**Job Role and Responsibilities**

**Duties include but are not limited to**:
Domestic cleaning, meal prep, Community access, Transport to and from medical appointments, Personal care, Manual Handling/ lifters and wheelchair transfers**.**

**Experience/Certification Required**
- Relevant qualification Certification III or IV in Disability/Aged Care/Individual Support/Social Work
- Full Victorian Driver's License and your own vehicle
- National Criminal Check
- Working with Children Check
- First Aid & CPR (or willingness to get one)
- Covid-19 Vaccination Certificate
- Manual Handling (not mandatory but preferable)

**Job Types**: Part-time, Casual

**Salary**:$32.00 per hour (weekdays)

Schedule:

- Shift work
- **Active Overnight ** Pay rate will differ from the above rate.**
