
Disability Support Worker

**Wellness Care Pty Ltd** is a registered NDIS provider committed to delivering person-centred, high-quality support services to individuals living with disability. We are currently seeking passionate, reliable and empathetic Support Workers to join our growing team.
As a Support Worker, you will play a vital role in assisting NDIS participants to live as independently as possible. You will provide in-home and community-based support tailored to each individual's goals, preferences and needs.
**Key Responsibilities**
- Assist with medication prompts and health-related routines
- Support participants in accessing the community (appointments, shopping, social outings)
- Encourage skill development, independence and participation in daily activities
- Build respectful and positive relationships with clients, families and stakeholders
- Accurately document progress notes and incident reports as required
- Follow participant care plans and Wellness Care’s policies and procedures
- Support with household tasks such as cooking, cleaning and laundry

- Ability to work independently and adapt to participant needs
- Availability for flexible shifts, including evenings and weekends
**Essential Requirements**
- Driver License
- WWCC
- Police check
- CPR
- First aid
- Comprehensive car insurance
- Latest Car registration
- NDIS Worker Check
- Qualifications - either aged care in individual support/certificate 3 or 4/Diploma
- NDIS worker orientation module
- Certificate infection control training
- Public liability insurance
