BaptistCare Community Services & Housing are currently recruiting a Tailored Support Coordinator to join the Carlingford Housing team on a permanent full-time basis. You will be responsible for providing effective assessment and planning to coordinate and support residents’ access to a range of individualised services to improve their well-being and achieve their goals in life.
- Min 3 years’ experience Aged Care, Disability Support, Social Housing, Community Services or related discipline
- Certificate IV - Social Work or Community Services (desireable)
- Demonstrated ability to facilitate the provision of effective and individualised support to achieve positive outcomes for tenants and household members.
- Following up on a periodic basis with tenants as needed and enable them to self-manage relationships with carers and family members.
- Experience working with high needs, complex clients
- High level negotiation skills and ability to liaise with key stakeholders
- Competence in service assessment and documentation (including service planning)
- NSW Driver’s Licence and NSW Working with Children check for paid employment purposes
- Previous experience working in community services or related discipline
- Knowledge of strengths based approach with excellent interpersonal skills
- Experience in community engagement and fostering community
