**Job description**
We are looking for experienced Care Managers with previous experience in client coordination, care plan and other care management responsibilities.
**Responsibilities and Duties**:
- Working with customers, their support and other providers to achieve their goals in line with their individual plans.
- Maintain and up to date case notes on customer progress accurately.
- Maintain an up-to-date knowledge of local services and pathways that customers can access and current sector systems and practices.
- Participate in accreditation standards and practices activities in conjunction with the Program Support Coordinator.
- Maintain internal filing, including client documentation and other relevant filings accurately.
- Complying with all relevant legislation
- Build relationships with relevant stakeholders to meet consumer needs and services
**Key Selection Criteria**:
- Be able to speak Cantonese
- Good understanding of relevant legislation and NDIS industry knowledge
- Demonstrated leadership skills and experience working with case management workforce within a community setting, managing complex client care scenarios
- Ability to effectively handle difficult conversations
- Advanced interpersonal and communication skills
- Ability to self-manage, prioritise work and handle multiple tasks
- Ability to prioritize and time manage multiple tasks to meet tight deadlines
- Good investigation and problem-solving abilities
- Capacity to respond to crisis situations using well-developed conflict resolution, negotiation and networking skills
- Excellent administrative and organisational skills
**Essential Criteria**:
- Relevant qualifications in case management, community and social service management or equivalent.
- Minimum 02 years experience in a similar role.
- Well-developed IT skills and experience with digital solutions.
- Unrestricted NSW Driver’s licence
- Be an Australian citizen or permanent resident
- Current First Aid certificate
- Working with Children Check
- NDIS Worker Screening Check
- National Police Check
**Job Types**: Full-time, Permanent
**Salary**: $65,000.00 - $90,000.00 per year
Schedule:
- Monday to Friday
Supplemental pay types:
- Bonus

- Lansvale, NSW: Reliably commute or planning to relocate before starting work (required)
