In particular, you will be responsible for the following:

- Assist in transitioning participants into Connecting Families care
- Supervise and monitor the maintenance of clinical & medical records and other care-related documentation for all NSW participants
- Review support plans and health assessments facilitating change as necessary to meet client needs
- Lodge, monitor and maintain submissions for all NDIS-related restrictive practice authorisations, and carry out monthly reporting of restrictive practice usage
- Prepare and develop transition plans and on-going monitoring

About you

As Assistant in Nursing and Clinical Support you are a strong problem solver as you have the ability to use your critical thinking skills to make decisions. You also have exceptional attention to detail and take initiative to work through cases and incidents.

Here is what else will set you apart:

- Demonstrated experience within a Nursing and clinical support role, or similar
- Excellent communication skills, both written and verbal
- Ability to work under pressure managing multiple priorities
- Assistant/Registered Nurse qualifications
