Gympie, Australia Superior Home Health Services Full time

**Superior Home Health Services** are seeking an enrolled nurse to support Aged Clients to access and utilise home care funding to stay in their homes safely. You will be supported by an experienced clinical team in the **Gympie Region.**

We are looking for a friendly, positive, and empathetic person who wants to make a difference.

**Responsibilities will include**:

- Working within a person-centered, enabling environment that respects the rights and needs of each client;
- Manage case management for a variety of aged clients;
- Manage Home Care Package funding administrative tasks;
- Undertake assessment and Care Planning activities;
- Refer clients to broader health providers where required;
- Provide support to access social and recreational activities;
- Observe and manage client deterioration and implement changes to care planning;
- Work under our Clinical Manager to undertake skilled nursing tasks;
- Respond to emergency situations and manage incidents.

**Essential Criteria**:

- Minimum Diploma of Nursing
- Excellent verbal and written communication skills
- Willingness to learn, develop and take on new challenges
- Current National Police Check
- Current First Aid & CPR certificates
- Current unrestricted driver’s licence
- Medication Assistance
- A reliable, well-maintained vehicle
- Comprehensive car insurance
- NDIS Worker Screening including Blue Card (or equivalent)
- Community experience, 1 year minimum (preferred)

**Job Types**: Full-time, Part-time, Permanent

Pay: $42.00 - $45.00 per hour

Expected hours: 38 per week
