**The Position**

We are seeking enthusiastic, passionate, caring female support worker to join our supportive and growing team to work with participant for community access within our organisation in Coburg.

Shift Days: Wednesday & Thursday

**Your responsibility include but are not limited to**:

- Community participation: accompanying participants on outings, appointments and social engagements
- Assistance at home: meal preparation and housekeeping
- Transporting participants to their required destinations
- Submit progress notes for every shift, following the template provided
- Building rapport and providing amazing services
- Assist in participants goals and aspirations

**Requirements**:

- Must have evident experience in Disability Care and the NDIS
- Fluent and strong verbal and written communication
- Positive attitude and takes initiative
- Able to work unsupervised and part of a team
- Updated First Aid & CPR
- Valid Driver's License
- Certificate III and/or Certificate IV in Disability
- Own car with comprehensive insurance
- Current Police Check
- Working With Children Check
- NDIS Worker Screener Check
- ABN
- NDIS Module Certificate
