This organisation primarily services private clients, NDIS patients, mental health, chronic disease, disability management, and neurological rehabilitation clients.

**About the Role**

Due to growth within the team and demand of services, our client is currently on the lookout for a passionate Social Worker to join their multidisciplinary team of clinicians.
- Support the planning and coordination of services for individuals with a disability living within the community.
- Connect clients to existing supports and services and also new ones in the community.
- Provide a high level advocacy service for clients, ensuring their needs and goals are met.
- Ensure clients are appropriately connected with the organisations various therapy services and work collaboratively within an MDT environment.

**Essential Criteria**
- Tertiary qualification in Social Work
- Registration with AASW
- Demonstrated postgraduate experience as a Social Worker
- A commitment to delivering person-centred, high-quality services for clients with neurological and developmental conditions.
- An ongoing desire to improve and achieve personal goals
- Australian Drivers Licence and reliable vehicle
- NDIS Worker Screening Check (or willingness to obtain)
