**This is a maternity relief position and should the permanent incumbent **require** an early return from **maternity leave**, this contract of employment will cease with two **weeks notice**. Maternity relief positions can be for a period of up to 12 months, with the possibility of continued employment for up to a further 12 months for maternity leave reduced hours, therefore there are no claims to permanency.**

**What you'll be doing**

The position of Bereavement Counsellor/Social Worker is part of a community based multidisciplinary Specialist Palliative Care Service (SPCS). The Service provides a community service approach, seeing clients and their families across the Central Coast. This position will work within a dedicated interdisciplinary team to deliver high quality care in a dynamic team environment.
Primarily the position is to provide skilled bereavement counselling to those people experiencing complicated grief, within an approved Model of Care Framework, using mixed approaches of face to face, video and telephone contacts. The position will also provide assessments and interventions to assist patients and their carers to deal with the emotional impact of illness and to undertake case planning, liaison and advocacy and referral to ensure access to appropriate community services. The social worker is to work closely with interdisciplinary colleagues and senior managers to ensure the development and delivery of effective and pro-active social work services. The social worker will be expected to be active in the areas of service development, service evaluation and staff and student teaching.

This position is required to work collaboratively with other social workers within the team to ensure appropriate and timely service provision across all areas of the Central Coast. The clinical duties are performed in a range of settings, including home, residential aged care facilities (when indicated), clinics, groups and is mostly based in Long Jetty.

**About You**
- Qualification of a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number) with demonstrated understanding of the current AASW Practice Standards as they relate to the position and a commitment to maintaining a professional skill and knowledge base.
- Demonstrated skills and knowledge in supporting people experiencing complex grief, utilising evidence-based therapeutic techniques and current grief models. Demonstrated well-developed understanding of the psycho-social impact of life-limiting health conditions and in the range of related social work interventions required in community social work clinical practice.
- A current Australian drivers licence.
