**About The Role**
The position within the Central Coast Mental Health Service (CCMHS) holds a pivotal role in the Adult Non-Acute Community Mental Health Teams, focusing on delivering essential services. Responsibilities encompass clinical care, including non-acute assessment and care planning for individuals with mental health issues. Collaboration with local general practitioners and other service providers is integral to developing sustainable support networks and fostering positive mental health outcomes for clients. Additionally, the role involves providing education, cultural expertise, and guidance for care planning, ensuring that adult non-acute community mental health services prioritize culturally appropriate and sensitive mental health care for clients accessing mainstream services.
**Key Accountabilities**
- **Quality Improvement and Customer Service**:
o Participate in quality improvement activities and provide high-level customer service to patients, staff, and other stakeholders.
- **Commitment to Vision and Values**:
o Demonstrate commitment to the Caring for the Coast vision, goals, and strategies, aligning behaviours with NSW Health CORE values and CCLHD Values and Behaviours Charter.
- **Holistic Client Care and Collaboration**:
o Manage an allocated case load, working with individuals, families, and the community to promote health and reduce illness risk.
o Collaborate with internal and external services to facilitate positive client outcomes, offering cultural liaison support where needed.
o Support clients in accessing services and educate carer/family supports, maintaining accurate clinical documentation and confidentiality.
**About You**
- Minimum Tertiary Bachelor Degree in Occupational Therapy or Counselling or Community Welfare or Social Work or Nursing, OR qualification as Aboriginal Health Practitioner (where qualification provides eligibility for professional membership of the relevant professional body e.g. AASW) plus general registration with AHPRA (where applicable).
- Demonstrated experience in the delivery of mental health care effectively and efficiently in a range of mental health settings, including a high level of interpersonal skills, communication skills, computer literacy with demonstrated experience in working in collaboration and partnership with multiple agencies in order to deliver holistic care.
- Knowledge of and demonstrated skill in providing culturally appropriate mental health assessment, treatment, support and advocacy; and knowledge and understanding of Aboriginal culture and the social and emotional issues experience by Aboriginal people.
