**About you**
The key to success in this role is the demonstrated ability in the below skills and experiences:
Case Management experienced in the National Disability Insurance Scheme with experience in developing and managing a budget for disability care Experience.
Ability to work autonomously and in a team environment Excellent time management & problem solving and communication skills with the ability to network.
Excellent communication and interpersonal skills to build rapport with various stakeholders and leadership
**Position Description**
This position will be responsible for leading the delivery of client services for Care Services' Disability services to ensure effective and positive outcomes for clients and their families. Ensures compliance with all aspects of Disability standards and all other legal and contractual compliance requirements.
You will also be expected to contribute to the strategic direction of the organisation including input and advice to support organisational strategy and objectives.
**The key duties include**:
- Consulting with participants and their families to assess care and support needs
- Organise with family/participants intake appointments and completing risk assessments
- Create care plans and budgets for participants
- Onboarding new participants
- Prepare service plan and roster care workers for participants
- Monitor and keep in regular communication with participants and their families
- Conduct reassessment of care plan for participants
- Arranging and coordinating support services for participants
- Increasing the value of current participants to attracting new ones.
- increasing number of participants

- Promotional activities
**Key Selection Criteria**
Essential Skills, Knowledge & Experience
- Capacity to respond to crisis situations, using well developed conflict resolution, negotiation and networking skills.
- Demonstrated capability to work across NDIS funding streams
- Demonstrated ability to operate independently and professionally with limited supervision.
- Capacity to effectively deliver and monitor service provision in a busy caseload within budgetary requirements.
- Advanced interpersonal and communication skills with demonstrated capacity to work with individuals to respond in ways that are meaningful, demonstrating active listening skills and empathy.
- Understanding of the principles of comprehensive assessment working from strengths based, client centred perspectives.
- Capacity to interpret and work within the relevant Policy guidelines.
- Knowledge of the needs of diverse groups, such as people from CALD and ATSI backgrounds, those identifying as LGBTI, Veterans, and the ability to respond in a sensitive manner to the unique needs of individuals.
- Excellent administrative and organisational skills.
- Experience in the same role
**Qualifications**
- Qualification relevant to care management, health or aged care or business with substantial experience in the community services area or business management in a related field.
- Qualification in workplace training and assessment is desirable.
**Other Requirements**
- Current unrestricted VIC Driver’s license.
- Required to maintain a current First Aid certificate.
- Must be an Australian citizen or permanent resident
- Required to hold a working with children permit.
- Required to provide a satisfactory national police check.
- Provision of a satisfactory pre-employment medical and fitness report.
- current car registration(you may need driving your car on clients visit)
- Australian Association of Social Worker (AASW) membership (preferred)

**Salary**: $76,000.00 - $83,700.00 per year
