As a Support Worker, you will be responsible for providing support to individuals with various physical, emotional, and social needs. Your role will focus on ensuring the well-being, independence, and dignity of those you support, while fostering an environment that encourages personal growth and positive outcomes.

**Key Responsibilities**:

- Assist clients with daily living tasks, including personal care, household chores, meal preparation, and medication management
- Support clients in developing life skills and achieving their personal goals
- Provide companionship and emotional support, creating a safe and supportive environment
- Assist with mobility and transportation to appointments or social activities
- Record and report client progress and any changes in their condition to supervisors
- Foster a positive, empowering relationship with clients and their families

**Key Requirements**:

- Certificate III or IV in Disability, Aged Care, or Individual Support (or equivalent)
- Previous experience in the community care sector is highly regarded
- Strong communication and interpersonal skills
- A caring, compassionate, and patient attitude
- Ability to work both independently and as part of a team
- Valid driver's license and reliable transport
- National Police Check and Working with Children Check
- First Aid and CPR certification
