**Work ONE DAY FROM HOME - 4 days in clinic**
**About the role**
We are looking for a new motivated and enthusiastic Key Worker to join our multidisciplinary team

Essential Criteria:

- Assisting parents/families to develop understanding and knowledge of the NDIS to support their ability to make informed decisions
- Plan, implement and evaluate individual therapy programs
- Complete documentation in line with professional standards
- Work collaboratively with patients, families, support staff, and other allied health services
- Write progress reports or supporting documentation for clients
- Identify and provide appropriate information and education to patients, families, support staff, and other allied health services
- Partake in regular meetings with other health professionals to ensure ethical practice

Duties include:

- Administering assessments
- Administering therapy sessions with clients and families
- Completing progress notes and reports
- Liaising with other professionals e.g. doctors, teachers, other therapists, work colleagues
