**The Role**

We are currently seeking a Support Worker Officer to join our organisation based in Taree region. As part of a person-centred team and individually, carry out lawful duties, directions (as per individual duty roster), and group/program/individual activities aimed at supporting and assisting vulnerable persons within our community and especially those with a disability in their living and/or community environment.

The role of support worker is also expected to work within and respect the professional boundaries of Many Rivers Support Services, as outlined within the relevant policies and procedures, as well as the individually signed professional boundaries document.

**Key responsibilities-**
- Responsible for providing professional, person-centred support and services to supported persons with particular reference to the supported person’s risk management profile and individual support needs. Be aware of, and responsive to, the changing needs of supported persons
- Responsible for ensuring that the people you support are given information on their rights and responsibilities, available services and the organisation’s complaints and feedback process
- Responsible for using the person-centred planning tools in partnership with supported persons and their representatives to learn about a person and keep learning about them
- Responsible for providing support with regard to and in accordance with a supported person’s individual goals, aspirations, beliefs, family situation, culture, age, sexual identity, gender, relationship status, disability type and language
- Responsible for providing support and service to people with a disability in accordance with the values of Many Rivers Support Services;
- Responsible for recognising and supporting individual and family relationships and connections to the community;
- Responsible for supporting the person with a disability achieve the goals outlined in their individual service plan;
- Responsible for supervising volunteers that are working on shift with you, including imparting relevant knowledge on supported persons and any other relevant work knowledge to assist volunteers to contribute meaningfully to service provision;
- Responsible for conducting person centred planning sessions with assigned supported persons and their families where requested. This includes the initial person-centred planning session and subsequent annual reviews;
- Responsible for providing support in a manner that allows people with a disability to develop and/or maintain life skills and encourages active and meaningful economic (if relevant) and/or social participation in the community;
- Responsible for facilitating opportunities for people with a disability to meet other people and develop positive friendships over time

- Responsible for evaluating activities and/or programs against a person’s needs and

Communicating any issues, either positive or negative, to the person’s representative and/or your direct line manager or other appropriate channel. This includes being able to suggest alternatives.
- Responsible for collecting, storing, supervising and documenting the administration of medication in accordance with the organisation’s medication management policy & practice
- Responsible for acting as a good social role model in terms of appearance, attitude, behaviour and manners;
- Responsible for completing any required documentation during your shift and submitting the documentation to the organisation;
- Responsible for submitting a timesheet each fortnight through the appropriate system(s);
- Take a positive attitude into the workplace, along with an understanding that flexible hours are a requirement of this position.
- Remember that Many Rivers Support Services is your first point of call for any questions relating to your role or the person you are caring for;
- Responsible for the administration of medication (as per training competency that is mandatory for this role)

- Certificate III in Disability Services or Equivalent
- Senior First Aid Certificate
