**The Role**
The Clinical Care / Case Manager position is responsible for the provision of high-quality case management services for Home Care Package clients by undertaking a comprehensive assessment of each client in collaboration with the client and their family or carer, determining a number of health and well-being maintenance and promotion strategies aimed at encouraging independence and assisting the client to maintain a quality of life.
The position is based in our Gledswood Hills office, and covers the wider Macarthur area with an “oversee role” for the Murrumbidgee Lachlan region staff.

**Duties and responsibilities include**:
- The position will be responsible for the delivery of Client and Clinical Care to clients.
- Monitored the client’s progress to ensure optimal outcomes are achieved.
- Maintaining client assessment and Care Plans and progress notes.
- Making regular visits to existing and new clients.
- Ensuring the provision of these services are always in line with the funding bodies' requirements;
- Ensuring WHS requirements are in place.
- Lead the team in providing ongoing education, team meetings, training and support to the team of Community Care Workers.
**About you**:
- Caring, compassionate & friendly
- Registration with AHPRA is essential.
- Demonstrated experience in Case Management especially Home Care Packages.
- Experience working in the aged care or disability sector.
- Demonstrated understanding of needs and issues of people with disability from CALD backgrounds.
- Demonstrated experience in needs assessment and management of the elderly.
- Demonstrated ability to supervise and develop Community Care Workers
- Understanding of work health safety in home and community settings.
- High level of communication and ability to work collaboratively is essential.
- Experience in working with the Aged Care Quality and Safety Commission requirements
- Experience in Proda, Procura, MAC & Alayacare is highly regarded
- Demonstrated high level verbal and written communication skills.
- Reliable & dependable
- Proven relationship management with all internal and external stakeholders, as well as good interpersonal, negotiation and problem solving skills
- Ability to work as part of a team
- Comfortable using technology (i.e. mobile devices) for accessing rosters & recording customer assessments & documenting feedback
**What you will need**:
- Current Registered Nurse registration with Australian Health Practitioner Regulation Agency (AHPRA)
- Evidence of continued professional development
- NDIS Worker Screening Clearance / Criminal Record Check
- Current CPR / First Aid Certificate
- Current Australian Driver’s Licence
- Reliable, registered and insured vehicle (comprehensive insurance is preferred)
- Previous experience in the community care or health/aged care sector (preferred)
- Proof of Vaccination (where relevant) i.e. COVID-19 vaccination certificate
